The lateral flow device became positive earlier ( =day 3) than the (1 -> 3)-beta-D-glucan and galactomannan assays ( day 5), with all samples positive by each assay on day 7.Item Continuous Versus Intermittent Infusion Of Oxacillin For Treatment Of Infective Endocarditis Caused By Methicillin-Susceptible Staphylococcus Aureus(2009-05) Hughes, Darrel W.; Frei, Christopher R.; Maxwell, Pamela R.; Green, Kay; Patterson, Jan E.; Crawford, George E.; Lewis, James S.; Hughes, Darrel W.; Frei, Christopher R.; Maxwell, Pamela R.; Lewis, James S.Infective endocarditis (IE) is the fourth leading cause of life-threatening infection in the United States and imposes significant morbidity and mortality. The American Heart Association guidelines for the diagnosis and treatment of IE do not address continuous-infusion (CI) oxacillin. This retrospective study compares outcomes between CI oxacillin and intermittent-infusion (II) oxacillin in the treatment of IE caused by methicillin-susceptible Staphylococcus aureus (MSSA). A total of 709 medical records were reviewed for inpatients with definitive IE treated between 1 January 2000 and 31 December 2007. Continuous data were analyzed by Student's t test or the Wilcoxon rank sum test. The chi-square test or Fisher's exact test was used to compare nominal data. A multivariate logistic model was constructed. One hundred seven patients met eligibility criteria for inclusion into the study. Seventy-eight patients received CI oxacillin, whereas 28 received II oxacillin. CI and II groups were similar with respect to 30-day mortality (8% versus 10%, P = 0.7) and length of stay (20 versus 25 days, P = 0.4) but differed in 30-day microbiological cure (94% versus 79%, P = 0.03). Sixty-three patients received synergistic gentamicin, whereas 44 did not. The gentamicin and no-gentamicin groups were similar with respect to 30-day mortality (11% versus 4%, P = 0.2) and 30-day microbiological cure (90% versus 89%, P = 0.8); however, times to defervescence (4 versus 2 days, P = 0.02) were significantly different. CI oxacillin is an effective alternative to II oxacillin for the treatment of IE caused by MSSA and may improve microbiological cure. This convenient and pharmacodynamically optimized dosing regimen for oxacillin deserves consideration for patients with IE caused by MSSA.Item The Effect of Telehealth on Autism Diagnosis and Evaluation Seen Through Patient Satisfaction(2023-04) Jafarnia, JordanThe onset of COVID-19 caused healthcare providers across the nation to incorporate telehealth measures into their daily practices. Many providers who worked in the realm of children with developmental disorders saw the implementation of telehealth as a means to tackle long waitlists and broaden access to care in their field. This study analyzes the use and feasibility of telehealth-based evaluations for autism diagnoses within the scope of patient satisfaction. A 14-item survey was developed and administered through REDCap in both English and Spanish. The survey was piloted from November 2020 to August 2022. The overall response rate was 17.3% and the feedback was largely positive. In addition to this study, an extensive literature review also provided supporting evidence for the feasibility of telehealth use in autism diagnosis and evaluations.Item The Effect of ADHD Diagnosis on Individual Identity Formation(2021-05) Thompson, MorganIdentity formation and the narratives we construct about who we are, why we perceive the world around us the way we do, and the lens through which we interpret our environment are all life long processes that are formed as we encounter experiences that shape us into the person that we have been, are, and will be. Categorization processes, including the diagnostic process, have a dramatic effect on how we perceive ourselves, as they define and associate us within broader social groups that we use to relate to and identify ourselves. Diagnoses shape our identities through a variety of mechanisms, including stigma, illness narratives, illness stereotypes, social factors, and institutions, and how we perceive our diagnoses has a dramatic impact on how we view ourselves. Mental health diagnoses have a particular impact on individual identity formation, as we view the mind as central to our individual identity. While identity is a well studied phenomenon, Attention Deficit Hyperactivity Disorder, or ADHD, is a fairly new diagnostic label. Not much literature has been dedicated to studying how the ADHD diagnosis affects the individual’s identity formation. Thus, my thesis enhances current research by describing the processes through which we form our identities, how we make sense of diagnoses, and the structures that form diagnoses in order to answer how ADHD diagnosis affects individual identity formation. After discussing the processes through which ADHD diagnosis is interpreted by the individual, my thesis discusses how to improve the diagnostic process and the ADHD label in hopes that refinement of these processes will benefit the individual. Study of how mental health and chronic diagnoses affect identity formation are essential for moral and social understanding of our diagnostic and social structures, as we should know how the diagnostic categories we create affect what it means to be us.
